FORM OF REQUEST

A requester may request access to records of Construction ID by filling out Form C, said form is available on request from the website of the Department of Justice –
www.justice.gov.za

The request for access must contain at the very least the following –

full particulars of the requester including an address in the Republic and fax and tel numbers sufficient details of the record requested for identification

which form of access is required

the right the requester wishes to exercise or protect together with an explanation of why the record is required for the exercise or protection of the right

If a requester is dissatisfied with the decision of the Information Officer or the request is refused, the requester may approach a court of law – magistrates’ court or high court for relief.
